Product Overview
DrFirst’s RcopiaAC for hospitals is a powerful, web-based application that collects, displays, and documents patient medication information from admission to discharge to facilitate medication reconciliation and to build your community health data warehouse. RcopiaAC can be used standalone or interfaced with your existing or future HCIS systems for patient ADT information.

Current Hospital Challenges
Achieving an efficient medication reconciliation process can be difficult. Lack of patient knowledge regarding medication details, multiple care providers, multiple sources obtaining different medication lists for the same patient and Joint Commission requirements all contribute to the difficulty. Many hospitals are finding it challenging to determine the best method to manage patient medications and thereby improve patient safety. Although the required steps of verification, clarification, and reconciliation may seem basic, managing the underlying logistics can be far more complex. Some of the challenges include:
  1. Patient safety risks associated with inadequate medication history lists to make good clinical decisions
    • Patient is a poor historian
    • Caregiver has limited knowledge
    • Incomplete documentation
    • Financial impact of misdiagnoses
  2. Manually procuring medication history from multiple sources is a time-consuming and costly process
  3. Inefficient maintenance of the medication reconciliation paper trail
  4. Accurate and complete discharge medications lists difficult to provide to the patient/caregiver
  5. Writing discharge medication orders that are "Formulary Compliant" and with a co-pay acceptable to the patient
  6. Meeting Joint Commission (JCAHO) Requirements
  7. Identifying in the Emergency Department a patient’s existing medications while providing prescriptions upon discharge
  8. Attracting and maintaining affiliated physicians
